ABSTRACT

Objective: This systematic review had the purpose to validate the probiotic Lactobacillus reuteri as adjuvant therapy in the periodontal treatment of periodontitis, by the analysis of randomized controlled trial, controlled clinical trial, and observational studies. Material and Methods: Search keys related to the subject were defined, and the following databases were used as search strategies: MEDLINE via PubMed, Scopus, Web of Science, Cochrane Central Controlled Trials Registry, and EMBASE. The data selection and study were performed by two independent evaluators: first, they selected the article by title and abstract and subsequently qualified according to the bias risk analysis. A narrative synthesis has been performed based on the data obtained from the best-quality articles. After data extraction, their heterogeneity was analyzed, and a meta-analysis was performed. Results: In general, the results of the meta-analysis were positive for the use of probiotics as an adjuvant treatment. Considering the limitations of the comparisons between the analyzed articles studied, the conclusion was that probiotics may provide supplementary benefits to the treatment of periodontitis, with improvement in bleeding on probing rates and probing depth (AU)


Objetivo: Esta revisão sistemática teve como objetivo validar o uso do probiótico Lactobacillus reuteri como terapia adjuvante no tratamento periodontal da periodontite, por meio da análise de ensaios clínicos randomizados, ensaios clínicos controlados e estudos observacionais. Material e Métodos: Chaves de busca relacionadas ao assunto foram definidas, e as seguintes bases de dados foram utilizadas como estratégias de busca: MEDLINE via PubMed, Scopus, Web of Science, Cochrane Central Controlled Trials Registry e EMBASE. A seleção dos dados e o estudo foram realizados por dois avaliadores independentes: primeiro, selecionaram o artigo pelo título e resumo e posteriormente qualificaram de acordo com a análise de risco de viés. Uma síntese narrativa foi realizada com base nos dados obtidos dos artigos de melhor qualidade. Após a extração dos dados, sua heterogeneidade foi analisada e uma meta-análise foi realizada. Resultados: Em geral, os resultados da meta-análise foram positivos para o uso de probióticos como tratamento adjuvante. Considerando as limitações das comparações entre os artigos analisados estudados, concluiu-se que os probióticos podem trazer benefícios complementares ao tratamento da periodontite, com melhora nas taxas de sangramento à sondagem e na profundidade de sondagem (AU)

ABSTRACT

Abstract Objective: This study assessed the efficacy of two adjunct therapies (antibiotic and probiotic) for periodontal treatment based on clinical and immunological parameters in patients with Stage II and III Grade B periodontitis. Methodology: 45 patients were randomly allocated into three groups: control group (CG); antibiotic group (GAtb), in which 500 mg amoxicillin + 400 mg metronidazole were used; and probiotic group (GProb), for which Lactobacillus reuteri was used. Patients received medications after undergoing periodontal debridement. Clinical and immunological parameters were assessed at baseline, 30 days, and 90 days. Results: All therapies reduced bleeding on probing (BoP) in the evaluated periods, and the GAtb had a greater reduction at 90 days (p=0.03). The GProb group showed better results for plaque index (PI) and gingival recession (GR) compared to the GAtb at 90 days (p=0.0014; p=0.006). The area of inflammation (PISA Index) significantly decreased in all therapies in the evaluated periods. Therapies had no significant differences regarding moderate pockets. The GAtb had a greater reduction in probing depth (PD) for deep pockets (p=0.03) at 90 days and in the number of deep pocket sites at 30 days (p=0.04). The occurrence of adverse effects was commonly reported in the GAtb as a percentage per patient. The GAtb had a significant reduction in the concentration of interleukins IL-1β and IL-8 and an increase in IL-10 and TNF-α. The CG had a reduction in IL-6 and IL-1 β, whereas in the GProb there was no difference. Conclusion: After three months, none of the adjuvant therapies provided any additional benefit for subgingival instrumentation.

ABSTRACT

Abstract Objective To compare two corticotomy surgical protocols in rats to verify whether they alter conventional orthodontic movement. Methodology Sixty Wistar rats were divided into three groups - orthodontic movement (CG), orthodontic movement and corticotomy (G1) and orthodontic movement with corticotomy and decortication (G2) - and euthanized after 7 and 14 days. Tooth movement (mm), bone volume fraction and bone volume ratio to total volume (BV/TV), and bone mineral density (BMD) were evaluated by micro-CT. The total amount of bone was measured in square millimeters and expressed as the percentage of bone area in the histomorphometry. The number of positive TRAP cells and RANK/RANKL/OPG interaction were also investigated. Results Day 14 showed a statistically significant difference in orthodontic tooth movement in CG compared with G1 (7.52 mm; p=0.009) and G2 (7.36 mm; p=0.016). A micro-CT analysis revealed a difference between CG, G1 and G2 regarding BV/TV, with G1 and G2 presenting a lower BV/TV ratio at 14 days (0.77 and 0.73 respectively); we found no statistically significant differences regarding BMD. There was a difference in the total amount of bone in the CG group between 7 and 14 days. At 14 days, CG presented a significantly higher bone percentage than G1 and G2. Regarding TRAP, G2 had more positive cells at 7 and 14 days compared with CG and G1. Conclusion Corticotomy accelerates orthodontic movement. Decortication does not improve corticotomy efficiency.

ABSTRACT

A estimulação elétrica (EE) tem sido usada no tratamento de lesões crônicas apresentando resultados favoráveis no fechamento da lesão cutânea entre outras aplicações clínicas. No entanto, a literatura carece de estudos avaliando se os efeitos da EE no fechamento da lesão oral. Diante disso, torna-se relevante investigar os efeitos da EE durante o processo de reparo da lesão oral em camundongos (Swiss). Métodos: Noventa animais foram divididos em grupos: Controle (C; n=45) que receberam a aplicação da EE com o aparelho desligado e Teste (EE; n=45) que receberam a EE (100 µA; 9 kHz; 660 mVpp; 1x/dia por 3 dias). Uma lesão de espessura total foi realizada com punch de biópsia de 1,5 mm de diâmetro. Os animais foram eutanasiados no 1o, 3o e 5o dia após a confecção da lesão. As amostras foram fixadas em paraformaldeído 4%, descalcificadas em ácido fórmico 20% e emblocadas em parafina. Os parâmetros avaliados foram: fechamento clínico da lesão; distância de reparação epitelial (DRE) e conjuntiva (DRC), concentração de citocinas IL-1ß, IL-6, IL-8, IL-10, TNF-α e VEGF; porcentagem de fibras colágenas e quantificação das proteínas de sinalização Smad2. Resultados: A área de fechamento clínico foi reduzida no grupo EE no 5º dia de avaliação (p=0,01). As DRE e DRC do grupo EE apresentaram menor distância em todos os tempos avaliados (p<0,05) com reduzida concentração de IL-6, TNF-α, IL-10 e VEGF (p<0,05). Não houve diferença na porcentagem de fibras colágenas e na phospho-Smad2. Conclusão: A EE teve efeito positivo nos parâmetros clínicos e histológicos com modulação da resposta inflamatória no início do processo de reparo da lesão(AU)


Background: Since Electrical Stimulation (ES) has used for chronic wounds treatment and other clinical applications showing favorable results in wound closure, it was hypothesized whether it could bring good results in oral wound closure. The aim was to investigate the ES effects during the oral wound healing process in mice (Swiss). Methods: Ninety animals were divided in groups: Control (C; n=45) which received a fictitious ES application, i.e. the ES applicance was shutted off and ES (ES; n=45) which received ES (100 µA; 9 kHz; 660 mVpp) once time a day for 3 days consecutively. A full thickness wound was done using a 1.5 mm diameter biopsy punch in the hard palate. The animals were euthanized on 1st, 3rd and 5th day post-wounding. The samples were fixed, decalcified and embedded in paraffin. The parameters evaluated were: clinical wound closure; distance of epithelial and connective wound edges (EWD and CWD); cytokines (IL-1ß, IL-6, IL-8, IL-10), TNF-α and VEGF by multiplex, percentage of collagen fibers and Smad proteins. Results: Clinical wound closure area was reduced on day 5 in ES group (p = 0.01). Both EWD and CWD distance were shorter in all times (p < 0.05) with a reduction of inflammatory cytokines production (p < 0.05). There were no difference in collagen percentage and phospho-Smad2. Conclusion: ES had a positive effect on clinical and histomorphometric wound closure outcomes showing a modulation of inflammatory response in the beginning of wound healing(AU)

ABSTRACT

Abstract Objective Considering the global public health problem of smoking, which can negatively influence bone tissue repair, the aim of this study is to analyze the influence of photobiomodulation therapy (PBM) on calvaria defects created surgically in specimens under the effect of cigarette smoke and analyzed with use of histomorphometric and immunohistochemistry techniques. Methodology Calvaria defects 4.1 mm in diameter were surgically created in the calvaria of 90-day-old rats (n=60) that were randomly divided into 4 experimental groups containing 15 animals each: control group (C), smoking group (S), laser group (L), and smoke associated with laser group (S+L). The animals were subjected to surgery for calvaria defects and underwent PBM, being evaluated at 21, 45, and 60 days post-surgery. The specimens were then processed for histomorphometric and immunohistochemistry analyses. The area of bone neoformation (ABN), percentage of bone neoformation (PBNF), and the remaining distance between the edges of the defects (D) were analyzed histometrically. Quantitative analysis of the TRAP immunolabeled cells was also performed. The data were subjected to analysis of variance (ANOVA) in conjunction with Tukey's test to verify the statistical differences between groups (p<0.05). Results The smoking group showed less ABN compared to the other experimental groups in all periods, and it also showed more D at 21 days compared to the remaining groups and at 45 days compared to the laser group. The smoking group showed a lower PNBF compared to the laser group in all experimental periods and compared to smoking combined with LLLT group at 21 days. Conclusions PBM acted on bone biomodulation, thus stimulating new bone formation and compensating for the negative factor of smoking, which can be used as a supportive therapy during bone repair processes.

ABSTRACT

Chronic renal disease promotes a decrease on kidneys filterability and nitrogen products accumulation on blood, electrolyte and system endocrine functions imbalance. Among the many clinical manifestations of chronic renal failure (CRF) in the oral cavity, there are: dry mouth, uremic stomatitis, radiographic changes in maxillary and jaw bones and accumulation of calculus on the teeth that increases levels of periodonto pathogenic microorganisms able to lead a periodontal tissue destruction and also have the potential to act from a distance on other organs, e.g. the kidneys. Thus, it becomes evident that a periodontal treatment of patients suffering from chronic renal failure is crucial for maintaining their general health conditions and a subsequent successful organ transplant.Thus, the objectives of this case report were to highlight how important the periodontal treatment is for chronic renal failure patients and to demonstrate improvements in their clinical condition through the treatment plan proposed herein. A patient with 43 years old carrier generalized moderate chronic periodontitis and CRF was submitted to scaling and root planning sessions, and later surgical access for scaling where the basic treatment not resulted in a resolution of the inflammatory periodontal process. The results obtained from a well-developed periodontal treatment and an effective cooperation of patient showed satisfactory results with periodontal disease process resolution or stabilization.Thus, periodontal treatment and patient compliance were crucial for the improvement of periodontal clinical conditions enabling a future successful renal transplantation. A doença renal crônica promove uma redução na filtrabilidade dos rins e produtos nitrogenados acumulam no sangue promovendo um desequilíbrio eletrolítico e funcional do sistema endócrino. Dentre as diversas manifestações clínicas da doença renal crônica (DRC) na cavidade bucal estão: xerostomia, estomatite urêmica, alterações radiográficas nos ossos maxilares e mandibulares e acúmulo de cálculo dental que eleva os níveis de microrganismos periodontopatogênicos capazes de promover a destruição periodontal e também a capacidade de agir à distância em outros órgãos, como por exemplo, os rins. Dessa forma, torna-se evidente que o tratamento periodontal de pacientes que sofrem com a doença renal crônica é crucial para a manutenção de sua saúde geral e para o subsequente sucesso no transplante de órgão. Então, os objetivos deste relato de caso foram evidenciar a importância do tratamento periodontal para pacientes renais crônicos e demonstrar a melhora na condição clínica através do plano de tratamento proposto. Um paciente de 43 anos de idade, portador de periodontite crônica moderada generalizada e de DRC foi submetido às sessões de raspagem e alisamento radicular, e posteriormente, acesso cirúrgico para raspagem onde o tratamento básico não resultou na resolução do processo inflamatório periodontal. Os resultados obtidos a partir de um tratamento periodontal bem conduzido e uma efetiva cooperação do paciente mostraram resultados satisfatórios com a resolução ou estabilização da doença periodontal. Desta forma, o tratamento periodontal e a cooperação do paciente foram cruciais para a melhora das condições clínicas periodontais possibilitando um sucesso no futuro transplante renal. (AU)

ABSTRACT

O objetivo deste relato foi apresentar um caso clínico no qual o levantamento do seio maxilar foi realizado em uma região com comunicação bucosinusal prévia decorrente da extração dos elementos 16 e 17. A comunicação bucosinusal foi fechada por um retalho mucoperiosteal no mesmo ato cirúrgico das exodontias. Após seis meses de cicatrização, o levantamento do seio maxilar foi associado ao enxerto ósseo bovino (Bio-Oss) e membrana reabsorvível (Bio-Guide). Depois de mais nove meses de cicatrização, dois implantes foram instalados pela técnica convencional, tendo perfeito travamento. As próteses foram confeccionadas após seis meses de osseointegração. A paciente foi acompanhada trimestralmente no primeiro ano e depois semestralmente. Passados três anos do término do tratamento, a paciente mostra-se satisfeita com os resultados. O protocolo apresentado neste caso para resolução da comunicação bucosinusal pós-extração dentária mostrou-se efetivo. Entretanto, o clínico deve considerar outras alternativas cirúrgicas, já que nem todas as perfurações apresentam as mesmas dimensões.


The objective of this report was to present a clinical case in which the maxillary sinus lifting was performed in a region with previous bucosinusal communication due to the extraction of elements 16 and 17. The bucosinusal communication was closed by a mucoperiosteal flap in the same surgical procedure of the exodontia. After six months of healing, maxillary sinus lift was associated with bovine bone graft (Bio-Oss) and reabsorbable membrane (Bio-Guide). After another nine months of healing, two implants were installed by the conventional technique, having perfect locking. The prostheses were made after six months of osseointegration. The patient was followed quarterly in the first year and then in the semiannual period. Three years after the end of treatment, the patient is satisfied with the results. The protocol presented in this case for resolution of oral buccal communication after dental extraction was effective. However, the clinician should consider other surgical alternatives, since not all perforations have the same dimensions.

Introduction: The number of hypertensive patients is increasing and prophylaxis with bicarbonate jet are widely performed in clinical practice using large amounts of this substance in a short period of time, which may lead to increased arterial pressure. In the literature there are several studies that analyze the effect of sodium bicarbonate jet on the biofilm and dental structures, but not report the effect on arterial pressure. Aim: Evaluated the change in arterial pressure before and after two procedures of dental prophylaxis, jet baking soda application and conventional prophylaxis, and patient opinion of the comfort of each system was obtained. Material and method: We selected 20 patients aged 18 to 30 in need of prophylaxis to remove biofilm. The patients were placed into three different treatment groups: sodium bicarbonate jet (G1), conventional prophylaxis (G2) and control (G3), with a one month interval between treatments. Patients were divided into groups randomly. Measurements were performed immediately before and after the procedure, 15 and 30 minutes after the end of treatment. Patient comfort was measured using a Visual Analog Scale (VAS) at the end of each treatment. Data were analyzed by analysis of variance. Result: There was a statistically significant difference in the comfort of the procedures, with G2 and G3 being better than G1. Additionally, an increase in the diastolic blood pressure was observed in sodium bicarbonate jet group evaluated just after the procedure. Conclusion: The conventional prophylaxis is more comfortable from the patient stand point and does not alter arterial pressure.


Introdução: A quantidade de pacientes hipertensivos está aumentando e, a profilaxia com jato de bicarbonato de sódio é bastante empregada na prática clínica usando grande quantidade dessa substância em um período curto de tempo que poderia aumentar a pressão arterial. Objetivo: Foi avaliado a alteração da pressão arterial antes e após os dois métodos de profilaxia dental e obtido a opinião de conforto do paciente em relação a esses métodos. Material e método: Selecionou-se 20 pacientes, na faixa etária de 18 a 30 anos, que necessitavam de profilaxia para remoção de biofilme dental e foram submetidos aos três tipos diferentes de tratamento: jato de bicarbonato de sódio (G1), profilaxia convencional (G2), e controle (G3), em intervalos de um mês entre eles. Os pacientes foram divididos de forma randomizada. As aferições realizaram-se antes da profilaxia, imediatamente ao fim do procedimento, 15 e 30 minutos após o término do tratamento. O conforto do paciente foi medido por meio de uma Escala Analógica Visual (VAS) após o final de cada tratamento. Os dados foram analisados por meio de teste de Variância. Resultado: Houve diferença estatisticamente significante quanto ao conforto dos procedimentos, sendo G2 e G3 melhores que G1. Observou-se aumento da pressão arterial diastólica no grupo jato de bicarbonato de sódio imediatamente após a profilaxia. Conclusão: A profilaxia convencional é mais confortável do ponto de vista do paciente e não altera a pressão arterial.
